The medical declaration

When a service requires it, every diver completes the official Diver Medical questionnaire (the universal DMSC form, v2022-02-01) as part of booking - the same form PADI, SSI and other agencies use. They sign it on screen. If their answers mean a doctor's sign-off is needed, that's flagged and the form is emailed for a physician to complete.

When this happens

  • Automatically, during the public booking flow, for any service marked as requiring a medical. Each diver in the party completes their own.

What the customer sees

  • A Diver Medical questionnaire step: the 10 standard health questions.
  • Answering Yes to a question that has follow-ups opens an extra set of questions (the form's "Box A-G") right underneath. Answering No again hides and clears them.
  • A participant statement they tick to confirm they have answered honestly. For an under-18, a parent/guardian name, relationship and signature are required instead.
  • A signature box they sign with a finger (on a phone) or the mouse.
  • A clear note if a doctor's sign-off is required - this happens if they answer Yes to question 3, 5 or 10, or to any follow-up question.
  • After booking, an email with their completed, signed Diver Medical form attached for their records. If a doctor's sign-off is required, the blank form (including the physician's evaluation page) is also attached to take to a doctor.

What you see in the dashboard

  • A "⚠ Medical" badge on the booking in the Bookings list when a physician sign-off is needed.
  • On the customer's profile, a Medical panel showing their latest questionnaire: the outcome (Cleared to dive or Physician sign-off required), the date they signed, their signature, and a link to view the completed signed form (PDF).
  • Each booking carries a medical status: not required, cleared, or needs physician.

Tips

  • Toggle whether a service needs a medical on the service itself in the Dive Menu.
  • The medical belongs to the person, so it stays on their record across bookings.
  • Signed forms and signatures are private - only your own centre's staff can open them, and they stay on the customer's record.
